   Together with the IT team of the Catharina Hospital Eindhoven, we - Philips Applied Technologies - developed our own custom made CDA Referral and Discharge notes. We used them in order to refer a patient treated by the Philips Motiva system to the Catharina Hospital Eindhoven. We have held numerous demonstrators around the Netherlands, while our solutions is already adopted by Catharina Hospital for their communication with the local General Practitioners.

      First we decided which sections we should include in the CDA Referral note (Reason for referral, medications, vital body measurements etc.). We used level 3 for lab results and vital types and level 2 for all the rest sections. This helped in more easily integrating the notes in the interconnected systems.
